About this template
A risk register laid out for a shipyard newbuild contract, with 15 sample risks already written so that a project manager or planning lead has a realistic starting list rather than a blank grid. It works for the yard side, and an owner's team can use the same structure to log its own exposure.
What's inside: one worksheet, "Risk Register", with a header for vessel, yard, overall risk owner and last-updated date, and a 12-column table: Risk ID, Category, Risk Description, Cause / Trigger, Probability (1-5), Impact (1-5), Score, Level, Mitigation / Response Plan, Owner, Status and Review Date. The sample risks span eight categories (schedule, resource, commercial, quality, design, procurement, HSE and regulatory) and cover typical newbuild problems such as late steel plate delivery, welder shortage at peak erection, owner design changes after steel cutting, main engine delivery slip, late owner-furnished equipment and commissioning delays that push the sea trial slot. The scoring key above the table sets the bands: Low 1-6, Medium 7-12, High 13-19, Critical 20-25.
How to use it: delete the sample rows that do not apply and add your own below. Score each risk for probability and impact, multiply them for the Score, and set the Level from the key. Give every risk a named owner and a review date, and walk through the open items at your regular project review, at least monthly and again after any major scope or schedule change.
Limits: Score and Level are calculated from probability and impact, and the Level colour follows the result, but only on the 15 sample rows; copy a row down when you add more risks. There are no dropdown lists for Category or Status.
