A decision log with one line per question for the Owner or the owner's representative: options and recommendation, who decides and who is consulted, the consuming schedule activity, needed-by date and last responsible date, schedule effect, status and written record. Formulas give days to needed-by, days decided late and a flag, with counters for open, overdue and schedule-affecting decisions.
